Introduction

The treatment of dental root channels is a complex process that requires the removal of microorganisms, especially Enterococcus faecalis, which is often the cause of treatment failure. Currently, the irrigation materials used in root canal treatment are not effective enough to overcome resistant bacteria such as E. faecalis. Therefore, further research is needed to find a better alternative, one of which uses ethanol extract from Siwak (Salvadora Persica). This study aims to examine the antibacterial effects of Siwak ethanol extract as an alternative to root canal irrigation material by finding minimal concentrations that can inhibit (KHM) and kill (KBM) E. Faecalis.

Background

Enterococcus faecalis is a pathogen that has a resistance to many antiseptic materials, making it a challenging bacteria to eliminate from root canal systems. The use of traditional irrigation materials, such as sodium hypochlorite and chlorhexidine, has been shown to be effective in reducing bacterial loads, but their effectiveness can be limited by the presence of biofilm and the development of resistance. Therefore, there is a need for alternative irrigation materials that can effectively eliminate E. faecalis from root canal systems.

Materials and Methods

This study used a posttest only control group design to examine the antibacterial effects of Siwak ethanol extract. The method used to test the antibacterial effect was the dilution method, where the siwak ethanol extract was diluted in Mueller Hinton Broth into several concentrations, namely 20%, 10%, 5%, 2.5%, and 1.25%. Each concentration was tested with five samples. The results of the study showed that the growth of E. faecalis in Siwak ethanol extract with a concentration of 20% showed a significant reduction compared to McFarland control. Meanwhile, for concentrations of 10%, 5%, 2.5%, and 1.25%, the results obtained show that bacterial growth cannot be calculated (tbud).

Results

The results of the study showed that the minimum concentration of Siwak ethanol extract that was effective in inhibiting the growth of E. faecalis was 20%. The KHM value was estimated to be in a concentration range of 10-20%. Statistical analysis conducted using the Kruskal-Wallis test and Mann-Whitney test showed that the Ethanol Siwak extract had a significant antibacterial effect on E. faecalis with a p-value of 0,0001, which showed results below 0.05. However, the KBM value in this study cannot be determined, because there is no extract concentration that can kill 99.9% of E. faecalis bacteria.

Discussion

The results of this study highlight the potential of Siwak ethanol extract as an alternative irrigation material in root canal care. Considering E. Faecalis is a pathogen that has a resistance to many antiseptic materials, the results that show antibacterial activity from Siwak ethanol extract provide new hopes in increasing the effectiveness of root canal care. Thus, siwak ethanol extract can be a suitable solution for the development of root canal irrigation materials that are more effective and environmentally friendly.
