This study aims to investigate the implementation of discharge planning for inpatients at the Hospital of the University of North Sumatra, with a focus on the challenges and recommendations for improvement.

The lack of time to provide education can lead to patients not fully understanding the process of further treatment after returning home, increasing the risk of health complications and readmissions.
