Of the total 40 subjects that meet the inclusion criteria, consisting of 26 men (66%) and 14 women (35%) with a value of P = 0.001. The analysis shows a significant difference in the average value of RI between groups of patients with and without AKI, each is 0.719 ± 0.060 and 0.060 ± 0.077 (p = 0.001). The RI parameters showed a sensitivity of 71%, 84% specificity, and 87% accuracy in predicting AKI incidence, with the area under the Receiver Operating Characteristic (AUROC) of 0.871. Conversely, NGAL has a sensitivity of 66%, specificity of 89%, and accuracy of 78% in early predictions of AKI events in patients treated at the ICU.

Analysis and Discussion

From the results of the study, it can be concluded that the RI value is higher in groups of patients who experience AKI compared to groups that do not experience AKI. This shows that RI has better ability to detect initial changes in kidney function than NGAL. Sensitivity and specificity that is higher than RI makes it a more appropriate tool for the prediction of AKI events in critical patients.

The advantage of using RI as a marker is a method that is not invasive and can be done quickly beside the patient's bed. Although not showing higher specificity, longer measurement time can result in delays in making important clinical decisions.

Conclusion

Both RI and NGAL have an important role in monitoring critical patients, but RI is superior as the initial predictor of AKI.
